A few words of welcome
To all of our readers - whether existing, new or prospective patients, or curious visitors - we welcome you.

The following pages contain information relevant to new and existing patients, including new patient forms, lists of insurance companies that are in and out of our network, methods of payment, cancellation policies, privacy policies, how to make appointments, and so on.

We ask that you review this information carefully. If you have visited our website in the past, you may find that there has been new information added since your last visit. If you would like to be apprised of significant changes to the site, we suggest you sign up for our newsletter

General guidelines
The following are a few general guidelines.
New patients are asked to fill out a copy of our new patient form and bring it to their first appointment. This helps to expedite the initial questions we need to ask when you first arrive, and gets you into your appointment more efficiently.
A parent, legal guardian or suitable responsible adult should accompany and remain with minors under the age of 18 for every appointment. That person is responsible for that child while with us.
Proof of insurance and/or payment is expected at each visit unless arrangements have been made well in advance of appointment day.
Our office has a strict 24-hour cancellation policy. With this in mind, GSFD reserves the right to charge for all missed appointments for which we were not given adequate notice.
Returned checks will result in a $30 fee.
